Active Home Health and Hospice was created with the goal of improving the standard of patient care in the home setting and actively serving those we care for in their journey through life. The ownership consists of local Registered Nurses and Healthcare Administrators that recognized a need in the community and wanted to make a positive impact on patient care. Although there are many Home Health and Hospice companies in Utah, we wanted to raise the bar by providing excellence in patient foc... used care.
