Update April 9, 2020

Effective Monday, April 13, 2020 we will resume operation.  All departments will resume operation with changes made to shifts and sanitizing procedures in place to comply with Indiana's guidelines.  While we are resuming normal operations a portion of our production will be devoted to producing N95 compliant masks for the State of Indiana.

Plan of Action

  • SHIFT CHANGES 
    • Production
      • 1st Shift
        • 6:30 a.m. to 3:00 p.m.
      • 2nd Shift 
        • 3:25p.m .to 11:55 p.m.
    • All other departments resume normal shifts unless notified by their immediate supervisor of a change
  • FACILITY SANITIZATION 
    • The entire facility has been fully sanitized during the shutdown and schedules for additional cleaning during operation time are in place
    • Additional sanitation equipment is in use to disinfect high traffic areas
    • Sanitation stations are set up throughout the facility for employee use
  • SOCIAL DISTANCING PRACTICES
    • Masks will be provided and recommended for all employees
    • Work areas that currently do not allow for appropriate social distancing have been re-configured to comply with guidelines
    • Break schedules are being adjusted to allow for more space

Medical Insurance Announcement

To remove any barriers for medically necessary testing, effective March 18, 2020 any member cost sharing for the COVID-19 test is waived (i.e. deductibles, copays, and coinsurance will not apply). Testing includes items and services furnished during health care provider office visits (including in-person visits and telehealth visits), urgent care visits, and emergency visits that result in an order for or administration of a test to the extent such items and services relate to the evaluation for the test and the furnishing or administration of the test.

All other treatment including but not limited to screening-evaluation that does not result in testing, hospital, transportation and pharmacy services will be covered in accordance with the existing terms and conditions set forth in the Summary Plan Description (i.e., no changes to treatment coverage).

401k Plan Ammendment Regarding COVID-19

Lafayette Venetian Blind has amended our 401K plan to allow for changes in distributions and
loans due to COVID-19. Our amendment is part of the recently passed CARES Act and will be in
