PACE at Home provides Elder Care, allowing people who require competent nursing home care to receive it. For each participant, PACE at Home develops a personalized care plan. The interdisciplinary team develops, reviews, and updates this person-centered plan of care regularly to ensure that each participant's requirements are met.
