TRANSITION PROJECTS
Transition Projects provides life-saving and life-changing services for people experiencing homelessness. They offer programs, resources, and tools to individuals through access to social services including caseworkers, healthcare, mentorship, and housing. In 2018 alone, more than 1,100 formerly homeless people found safe, affordable housing in the four-county area with their agency’s support.
